Overview

The National Notifiable Diseases Surveillance System (NNDSS) collects and collates information on over 70 communicable diseases or conditions that are nationally significant in Australia. This form is used by health professionals to notify relevant public health authorities of suspected or confirmed cases of these diseases.

All notifications are de-identified to protect patient privacy and are submitted in accordance with state and territory public health legislation. Accurate, timely reporting supports disease monitoring, outbreak response, and policy development at both the state and national levels.

 

How to Use This Form

Who Should Complete the Form:

  • This form is intended for use by medical practitioners, laboratories, and other authorised health professionals.

  • It can be used for both initial notifications and follow-up submissions if additional laboratory or clinical data becomes available.

Filling Out the Form:

  • Complete Section 1 with information about the reporting professional.

  • Provide de-identified patient information in Section 2, including age group, sex, location (postcode), and clinical/lab details.

  • Select the appropriate notifiable disease in Section 3.

  • Add any relevant contextual information, including hospitalisation or outcome, in Section 4.

What Not to Include:

  • Do not include names, Medicare numbers, or any personally identifying information about the patient.

Submission:

  • Submit the completed form through your state or territory’s preferred reporting channel, such as an online portal, secure email, or integrated practice software.
